Mills Pond Nursing and Rehabilitation Center is located in St. James, New York, on the North Shore of Long Island.
St. James got its name in the mid-19th century, when residents, then mostly farmers, adopted the name of the local Episcopal church, which still exists.
In the early 20th century, St. James was a popular vacation spot for glitterati of the stage and screen. Summer residents included Lionel, Ethel, and John Barrymore, Buster Keaton, Myrna Loy, and Irving Berlin, among others.
Noteworthy residents of the town have included Stanford White, architect; William Jay Gaynor, New York State Supreme Court Justice and Mayor of New York City; Willie Collier, actor and playwright; Mick Foley, Professional Wrestler; and Axella Johannesson, musician.
Many 19th century structures are located along New York State Route 25A, known locally as North Country Road. Among them is the Saint James Church, the source of the hamlet's name back when the 1873 St. James Rail Road Station was established. Although small, the historic station-house is the second-oldest on the Long Island Rail Road.

At Mills Pond Nursing and Rehabilitation Center, our mission is to provide the highest caliber of health care, rehabilitative and educational support services to our residents, their families and the community. A committed, dynamic team of case managers, physicians, dieticians and nutritionists, social workers, recreation therapists, registered nurses, licensed practical nurses and certified nursing assistants work together to optimize the healthcare outcomes of each patient.
Our recreation department host a variety of classes, events and workshops year-round. We are getting more and more excited about everything that will be going on here at the Mills Pond Nursing & Rehabilitation Center! We’ve got plans for everyone! Kids, adults and seniors. We hope you find something fun with us! We have added quite a bit of new recreational programs to our lineup this year and it’s great to see the interest that these new programs have received! We are always looking into trends and things that we think our community might like to see.
